The following forms and documents are for the use of CQP candidates and sponsors at various stages during the Program. To access the forms in French, please click on "Français" at the top right of your screen.

• Sponsor Recommendation Form

(This form must be completed by sponsors of candidates requesting exemption from the CQP General Entrance Requirements)

•  Sponsor Declaration Form

(Sponsors must sign this form attesting to their agreement to sponsor their candidates throughout the Program) (Candidates must also use this form when changing sponsors) 

•  Sponsor's Manual

(A guide for Sponsors on their role and responsibilities, etc)

•  Experience Report 

(Candidates are to complete and submit annually, attesting to the number of hours of experience gained in each required category)

• Report of Course Assignment Results - Introductory Course

(Sponsors must complete and submit this form once they have corrected the candidates’ assignments. Candidates who do not have a sponsor yet are to contact the Registrar for instructions)

• Self-Administered Exam Results Form - Introductory Course

(Sponsors must complete and submit this form once they have corrected the candidates’ exam. Candidates who do not have a sponsor yet are to contact the Registrar for instructions)  

• Report of Course Assignment Results - Core Knowledge Course 

(Sponsors must complete and submit this form once they have corrected the candidates’ assignments)    

• Report of Course Assignment Results - Applied Knowledge Course

(Sponsors must complete and submit this form once they have corrected the candidates' assignments)
